The Hospice By The Bay Team

Every day, patients and their families tell us how much it means to have the compassionate care and dedicated attention of our skilled Hospice By The Bay Team. Our Team works with the patient’s own physician, family members and caregivers to see that the patient’s choices about care are respected and that comfort and quality of life are maximized.

To meet the needs of both the patient and family, the Hospice By The Bay Team includes:

  • Physician Medical Directors to oversee the patient’s plan of care.
  • Nurses specially trained in end-of-life comfort care, who visit regularly and provide medical equipment, medication and treatments. They are available by phone 24-hours-a day, to answer questions or make an emergency visit.
  • Speech, physical, occupational or nutritional therapists, if needed.
  • Social Workers who educate and answer questions about hospice services, insurance, financial and legal issues, community resources, managing stress, and planning for the time of death.
  • Spiritual Support Counselors to give comfort and counseling based on the patient’s or family member’s own faith tradition.
  • Home Health Aides to help the patient with personal care needs, such as bathing and dressing.
  • Trained Volunteers who offer companionship, respite for family caregivers and other care that complements the work of professional staff.

To support the community and families with a loved one who is or has been in our care, Hospice By The Bay also offers:

  • Professional Grief Counselors to help anyone in the community who is coping with loss.
  • Community Education Programs, including Advance Care Planning and group presentations on hospice care.
