Hospice By The Bay Teams visit patients in Skilled Nursing Facilities and Residential Care Facilities for the Elderly. We value our partnerships with facility staff. This important relationship allows Hospice By The Bay to provide their residents additional services at the bedside and thier faciilty staff education in end-of-life care. Together, we can ensure that our mutual patients have the highest possible quality of life and their families have support during a difficult time.
While in our care, Hospice By The Bay can offer residents and their families these additional services:
Hospice Team members who reflect the cultural diversity of our communities and are fluent in languages such as Spanish or Chinese
Nursing support with end-of-life expertise and 24-hour-a-day access, resulting in less-frequent hospitalizations
Social Worker case management support
Home Health Aide personal care support
Spiritual counseling and outreach
Patient Care volunteers, providing personal support for residents and families
Vigil volunteers, trained to comfort residents who are in the "active" dying process
Grief counseling for family members of all ages
