AIDS Case Management Program

Hospice By The Bays AIDS Case Management Program provides a continuum of high quality, in-home skilled nursing, attendant care, case management, and psychosocial services to disabled persons with HIV/AIDS. The AIDS Case Management Program is funded by:

  • The State of California, Dept. of Health Services, Office of AIDS;
  • County of Marin, Dept. of Health & Human Service, Care Act Funds; and
  • Hospice By The Bay

Who is Eligible?
Services are available to Marin County residents diagnosed with disabling HIV or AIDS.

How Does the Program Work?
Persons with disabling HIV/AIDS may request services directly or be referred by a physician, community agency, or hospital discharge planner.

The AIDS Program Manager will arrange an intake visit. The client's personal physician will then be contacted to obtain certification of their diagnosis. During enrollment in the AIDS Program, each client, in collaboration with their personal physician, actively participates in developing a personal care plan.

Client satisfaction with program services is regularly monitored to ensure that a high level of quality is maintained.

What Services Are Available?
The AIDS Case Management Program provides a wide range of services through a multidisciplinary team.  Services can include:

  • In-home attendant care
  • MediSet preparation
  • Limited nursing services
  • Access to other community resources
  • Emotional support
  • Mental health referral
  • Benefits counseling referral
  • Transportation assistance
  • Food assistance
